A Case of Invasive Pulmonary Aspergillosis caused by Aspergillus niger

Abstract
Invasive aspergillosis is a commmonly encountered opportunistic fungal infection in immunocompromised patients. But Aspergillus niger is known to be limited to the causative organism. We examined a case of invasive pulmonary aspergillosis caused
by
A.
niger in a neutropenic patient who received radiotherapy, anti-cancer chemotherapy, and steroid treatment. There was no evidence of funal infection at blood culture and routine bacterial culture with sputum. In bronchial washing specimens,
specific
microscopic feature and colony morphology of A. niger was observed at fungus culture and dispersed or clustered black conidia was demonstrated at cytologic smears stained with H&E and Gomori methenamine silver. We suggest that fungal culture
should
be
done immediately when fever or pulmonary infiltrates do not respond to antibacterial therapy in neutropenic immunocompromised patients, and bronchial washing is more useful than sputum to detect invasive aspergillosis and that cytology slides are
to be
reviewed if culture positive. We report this case with brief review of literature(Korean J Clin Pathol 1995;15(1):112~20).
